If you've forgotten your credentials or been locked out of your account, this guide walks you through every way to reset your password in DigitalPatientChart — whether you're a clinician, front-desk staff member, or practice administrator, and whether you're on a desktop browser or a mobile device.

Reset Your Password

This is the standard self-service flow for any DigitalPatientChart user who has forgotten their password or can no longer log in. It covers the most common scenario: your registered email address is still active and accessible.

  1. Go to your practice's DigitalPatientChart login page.
  2. Click Forgot Password? beneath the login fields.
  3. Enter the email address associated with your account and click Send Reset Link.
  4. Open your email inbox and look for a message from [email protected] with the subject line "Reset Your DigitalPatientChart Password."
  5. Click the Reset Password button inside the email. The link is valid for 60 minutes.
  6. Enter a new password, confirm it, and click Save Password.
  7. Return to the login page and sign in with your new credentials.

If you accessed your online medical records account through a third-party practice management or billing system (for example, via a single sign-on tile), navigate directly to the DigitalPatientChart login URL rather than launching through the integrated system. The standard reset screen may not be reachable from within an integrated portal session.

Troubleshooting

Password reset email not showing up in your inbox

  • Check your Spam, Junk, and Promotions folders.
  • Add [email protected] to your contacts or safe-senders list, then request a new reset link.
  • Allow up to five minutes for delivery; email routing delays are uncommon but do occur.
  • Confirm you entered the correct email address — a single typo will route the email to a non-existent address with no error shown.

Why is my password reset link not working?

  • Reset links expire after 60 minutes. If the link has expired, return to the login page and request a new one.
  • Each new reset request invalidates all previous links. Use only the most recent email.
  • Some email clients wrap long URLs and break them. Copy and paste the full link directly into your browser's address bar rather than clicking it.

Account locked out — how to fix it

After five consecutive failed login attempts, DigitalPatientChart temporarily locks the account as a security measure. Wait 15 minutes and try again, or use the Forgot Password flow above to reset immediately.   1. Log in to DigitalPatientChart with your administrator credentials.
  2. Navigate to Settings > User Management.
  3. Locate the staff member's account using the search bar.
  4. Click the account row to open the user detail panel.

  6. To unlock a locked account without a password reset, click Unlock Account in the same panel.

Multi-location practices: If a staff member is assigned to more than one location, their account is tied to a single login. Resetting the password from any location's admin panel updates credentials system-wide — you do not need to repeat the process per location.

Resetting Your Password on a Mobile Device or Tablet

You can reset your DigitalPatientChart password on a phone or tablet using any modern mobile browser (Chrome, Safari, Firefox). The mobile browser experience mirrors the desktop flow, though the layout is condensed.

  1. Open your mobile browser and navigate to your practice's DigitalPatientChart login URL.
  2. Tap Forgot Password? — it appears below the password field. If you don't see it, scroll down; it may be hidden below the keyboard.
  3. Enter your email address and tap Send Reset Link.
  4. Switch to your email app, open the reset message, and tap the Reset Password button.
  5. Your mobile browser will open the reset form. Enter and confirm your new password, then tap Save Password.
  6. Return to the login page and sign in.

Tablet tip: If you normally use a saved bookmark or a home-screen shortcut to log into the patient portal on a tablet, make sure the shortcut points to the correct practice URL before starting the reset process. Outdated shortcuts occasionally route to a cached or incorrect login page.

If Your Registered Email Address Is No Longer Valid

A changed email address is one of the most common reasons users find they can't access their medical portal account — the standard reset link has nowhere to go. Self-service is not possible in this situation; account access requires identity verification by a staff member or administrator.

For patients: Contact your provider's front-desk team directly. Staff can update your email address in DigitalPatientChart after verifying your identity in person or by phone, then trigger a fresh password reset to your new address.

For clinic staff: Contact your practice administrator. They can update the registered email in Settings > User Management and issue a new reset link from there.
