Your Right to Request Amendments
Under HIPAA, you have the right to request that a healthcare provider amend your medical records if you believe they contain inaccurate or incomplete information.
How to Request a Correction
- Identify the specific record and the information you believe is incorrect.
- Go to Records and open the document in question.
- Click Request Amendment.
- Describe the error and provide the correct information.
- Submit your request. The provider has 60 days to respond.
Possible Outcomes
- Approved — The record is updated with the corrected information.
- Denied — The provider explains why the amendment was denied. You have the right to submit a statement of disagreement that will be attached to the record permanently.
Note: Providers cannot alter the original clinical notes. Amendments are appended as addenda to the original record.
