Your Right to Request Amendments

Under HIPAA, you have the right to request that a healthcare provider amend your medical records if you believe they contain inaccurate or incomplete information.

How to Request a Correction

  1. Identify the specific record and the information you believe is incorrect.
  2. Go to Records and open the document in question.
  3. Click Request Amendment.
  4. Describe the error and provide the correct information.
  5. Submit your request. The provider has 60 days to respond.

Possible Outcomes

  • Approved — The record is updated with the corrected information.
  • Denied — The provider explains why the amendment was denied. You have the right to submit a statement of disagreement that will be attached to the record permanently.

Note: Providers cannot alter the original clinical notes. Amendments are appended as addenda to the original record.
