Required Information
- Full legal name (must match your insurance card and government ID)
- Date of birth
- Phone number and email address
- Primary address
- Emergency contact (name, relationship, phone number)
Optional Information
- Insurance details (carrier, policy number, group number)
- Preferred pharmacy
- Primary care provider
- Known allergies and current medications
- Profile photo
Editing Your Profile
Navigate to Profile > Edit Profile from your dashboard. Changes save automatically. If you need to update your legal name, contact support with a copy of your updated ID — we verify name changes to protect your account security.
