Before Your Visit
- Complete all intake forms — Check your Medximity Forms section for pending paperwork.
- Bring your insurance card — Even if it's on file, bring your physical card to in-person visits.
- List your medications — Include prescription drugs, over-the-counter medications, and supplements with dosages.
- Write down your questions — Prepare a list of questions or concerns to discuss with your provider.
- Bring a government-issued ID — Required for new patient visits at most practices.
At the Office
Arrive 15 minutes early for new patient visits to allow time for any remaining paperwork and check-in. For established patient visits, arriving 5 minutes early is usually sufficient.
Bring Relevant Records
If you're seeing a new provider, bring or arrange transfer of:
- Recent imaging (X-rays, MRI, CT scans)
- Lab results from the past 12 months
- Referral letters from other providers
- Surgical history summaries
