How Records Are Delivered
When a provider fulfills your records request, the documents are delivered through the Medximity platform:
- You receive a notification (email and in-platform) when records are ready.
- Records are available for download from your Records dashboard.
- All files are delivered over encrypted connections.
- Download links expire after 30 days — download promptly.
Chain of Custody Documentation
Medximity automatically maintains a chain of custody log for every records request:
- Date and time the request was submitted
- Date and time the provider acknowledged the request
- Date and time records were uploaded by the provider
- Date and time you downloaded the records
- Identity of all parties involved (verified accounts)
This log is available for download as a PDF from the request detail page — useful for court filings and evidence authentication.
Document Integrity
Records delivered through Medximity are the original digital files from the provider's EHR system. They have not been modified by the platform. If you need a provider to certify records as true copies, you can request a certification through the platform.
