Essential Documents

Healthcare practices must maintain several legal documents. Medximity provides templates and management tools for:

  • Notice of Privacy Practices (NPP) — Required by HIPAA. Must be provided to every patient.
  • Informed consent for treatment — Patient authorization before procedures.
  • HIPAA authorization forms — For releasing records to third parties.
  • Financial agreements — Payment policies, cancellation policies, and fee schedules.
  • Telehealth consent — Required before conducting virtual visits.
  • Assignment of benefits — Authorizing insurance payments to be made directly to the provider.

Digital Document Management

DigitalPatientChart includes a forms system where you can create, assign, and track consent documents. Patients sign digitally through their portal, and signed copies are stored in the patient's record with timestamps and audit trails.

Record Retention

Federal and state laws require retaining medical records for specific periods (typically 7–10 years for adults, until age 21 or longer for minors).
