Before You Submit
You must have a valid HIPAA authorization signed by the patient (your client) that:
- Identifies you and your firm as the authorized recipient
- Specifies the provider authorized to release records
- Describes the information to be released (or states "complete medical records")
- States the purpose of the release (litigation, insurance claim, etc.)
- Includes an expiration date or event
- Is signed and dated by the patient
Submitting a Request
- Navigate to the provider's profile or go to Records > New Request.
- Upload the signed HIPAA authorization (PDF or image).
- Specify the records requested (date range, record types).
- Provide the patient's full name, date of birth, and any known patient ID.
- Add any special instructions or rush request notes.
- Submit. The provider is notified immediately.
Expected Timelines
Under HIPAA, providers must respond to records requests within 30 days (with one 30-day extension if needed). Most Medximity providers fulfill requests within 5–10 business days.
Record Fees
Providers may charge reasonable fees for copying and delivering records, as permitted by state law. Fee schedules are displayed during the request process.
